Please list ALL adults that will be given permission to attend SCCH with your child(ren). As stated in the child protection policy, all adults remaining on campus will have a background check completed by staff before the first day of co-op. Listing the names below gives SCCH permission to complete the background checks to ensure the safety of all involved in SCCH. You may not list anyone who currently attends co-op.
WAIVER: In consideration of my child(ren), ward, myself, and my family, being permitted to participate in any way, in any and all activities of the Secret City Christian Homeschoolers (SCCH) and Glenwood Baptist Church (GBC), at the residence of any Member or at any and all locations where such activities take place (collectively, the Activities), I, for myself, my child, my family, and/or ward, my heirs, personal representatives or assigns, do hereby covenant not to sue, and release, waive, and discharge, SCCH, and GBC, its directors, officers, staff members, volunteers, advisors, property owners, and/or agents, from liability and of and from any and all claims including the negligence or default of SCCH and GBC, its directors, officers, staff members, volunteers, advisors, property owners, and/or agents, resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in the Activities.

ASSUMPTION OF RISK: Participation in the Activities carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from (1) minor injuries such as scratches, bruises, and sprains, (2) major injuries such as eye injury, loss of sight, joint or back injuries, broken bones or teeth, asthma or heart attacks, and concussions, to (3) catastrophic injuries including paralysis and death.
I have read the previous paragraphs, and I know, understand, and appreciate these and other risks that are inherent in the Activities, which may include without limitation, parties, picnics, hiking, swimming, field trips to museums, plays, parks, recitals, or practices. I hereby assert that my participation and/or the participation of my children, family or wards, is voluntary, and that I knowingly assume and accept all risks. I also understand that I may participate in activities that are not sponsored or supported by SCCH and GBC but are private activities of Members and that I assume all risks associated with such private activities (non-SCCH/GBC activities).

INDEMNIFICATION AND HOLD HARMLESS: I also agree to INDEMNIFY AND HOLD SCCH/GBC, its directors, officers, staff members, volunteers, advisors, property owners, and/or agents, HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorneys’ fees, brought as a result of my involvement in the activities of non-SCCH/GBC activities or both, and/or the involvement of a minor child, family member or ward of mine in the Activities or non-SCCH/GBC Activities, or both, in any location where activities are conducted, and of and from all claims which may hereafter develop or accrue to them on account of injury, loss, or damage, which may be suffered by said minor or to any property, because of any matter, thing or condition, negligence or default whatsoever, and to reimburse them for any such expenses incurred.

SEVERABILITY: The undersigned further expressly agrees that the foregoing waiver, and assumption of risks, indemnification and hold harmless, agreement is intended to be as broad and inclusive as is permitted by the law of the State of Tennessee, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

ACKNOWLEDGMENT OF UNDERSTANDING: I have read this Waiver of Liability, Assumption of Risks, and Indemnity Agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

DATES COVERED BY WAIVER: All dates between July 1st 2025 and June 30th 2026.
* We acknowledge that we have read the SCCH rules and regulations and agree to abide by them. We understand it is at the sole discretion of the SCCH leadership board to refuse admission to an applicant or discontinue enrollment of an individual if the activities or behavior of the individual are in counter to, or opposition to, the Statement of faith. This includes, but not limited to, participating in, supporting, or condoning sexual immorality, homosexual or bi-sexual activity, or refusal to follow any of the SCCH rules.

* We have read through the waiver of liability and adhere to the terms listed that we will not hold SCCH, GBC, or any families involved liable in the event of any incident.

* We have read the child protection policy and understand our part in helping ensure the safety of all involved in SCCH and agree to act in an appropriate manner towards all individuals.

* We understand the conflict resolution plan and will utilize that when needed rather than gossiping or slandering the co-op or anyone in it.

* We understand a parent/guardian must stay on campus with our child(ren) and agree to accompany or have another adult accompany our minor child(ren) at all times while they are involved in any co-op activities on the premises of any Member, or at any other location, and acknowledge that we are fully and totally responsible for our child/ren at all times while he/she is participating in any Activities at SCCH/GBC or at any other location. We understand that in order to keep the cost of the co-op as minimal as possible and to run efficiently, a parent/guardian will be required to help in some capacity when our child(ren) is participating in classes. We will honor our commitment in the volunteer roles that are selected and do them to the best of our ability. We will strive to honor our commitment to the co-op with good attendance and will notify leadership when unable to attend. We also understand that in order to keep the safest environment possible for all of the kids, every adult that will be remaining on campus will have a background check completed by the co-op staff.
